New Patient Intake Process – Required Before Scheduling
We do not schedule new patient appointments until completed intake forms are received and reviewed.
Step 1: Download the new patient forms from our website
Step 2: Complete the forms
You may fill them out on your computer, or
Print and complete them by hand
Step 3: Email completed forms to: drdavidnebbeling@michiganregenhealth.com
After Review:
Once your forms are received and reviewed, our office will contact you to schedule an appointment.
If you have not heard from us within a reasonable timeframe, you may call the office to check on the status or proceed with scheduling.
If you do not have access to a printer or computer:
You may use a local public library or print/scan service. Most libraries can assist with printing, scanning, and emailing documents.
Important:
We do not schedule new patients by phone prior to receiving forms
We are unable to provide medical advice to individuals who are not established patients
If you are experiencing an urgent medical issue, please seek care at an urgent care center or emergency department
