New Patient Intake Process – Required Before Scheduling

We do not schedule new patient appointments until completed intake forms are received and reviewed.

Step 1: Download the new patient forms from our website

Step 2: Complete the forms

  • You may fill them out on your computer, or

  • Print and complete them by hand

Step 3: Email completed forms to: drdavidnebbeling@michiganregenhealth.com

After Review:

Once your forms are received and reviewed, our office will contact you to schedule an appointment.

If you have not heard from us within a reasonable timeframe, you may call the office to check on the status or proceed with scheduling.

If you do not have access to a printer or computer:

You may use a local public library or print/scan service. Most libraries can assist with printing, scanning, and emailing documents.

Important:

  • We do not schedule new patients by phone prior to receiving forms

  • We are unable to provide medical advice to individuals who are not established patients

  • If you are experiencing an urgent medical issue, please seek care at an urgent care center or emergency department

  • Incomplete forms may delay scheduling
